Need Help Dr. Anuj

39 year old male with the following symptoms. Used Prozacc for 5months which helped decrease his suspicion but the symptoms returned after stopping. Had covid a year back. Currently for the last 8 months having body weakness and feeling heaviness in the head upon waking up, dizziness, leg pain, swelling in both ankle, back pain, chronic cough with sputum (cough has increased), chest pain, shortness of breath while walking and laying down, bloating, intermittent stomach pain, anxiety, mild fever that sometimes comes and goes, chest pain. He is having difficulty walking which has increased so for past week has been taking taxi to work. Also having difficulty sleeping due to these symptoms and also seeing ghost when he wakes up in the middle of night so he is keeping the lights on at night. Living separately from wife, although wife tries to contact him to ask about his health, pushes him to seek treatment, he doesnt listen, needs a lot of pushing. He doesnt talk much anymore with wife, tries to avoid, wants to be left alone, hesitant to live together. Before he used to talk everyday but stopped after accusing her. On echocardiogram from 3years back shows Mild Mitral valve prolapse and trace triscuspid and mitral valve regurgitation. Recent xray shows misaligned back bone which doctor thinks maybe causing pain and prescribed methyl salicylate 280mg which he started to take. He is very lazy to go to doctor said he will go if he feels better which doesnt make sense as people go when they are sick.

-Suspicious of wife (looks for pattern if something feels odd gets suspicious and checks and seeks reassurance, child like behavior, wants to check phone, wants proof to feel better)
-Overreacts
-occasionally things seems like insult to him or thinks people are making fun of him especially within family if someone says something or posts something. He is normal with friends/outsiders.
-scolds yells inappropriately
-quick to anger, will stop talking for weeks on phone if get upset or suspicious.
-headache
-attention deficit
-wants to control
-repeats alot
-gets upset, mood changes
-doesnt like to exercise like to sit alot
-curses and disrespects, says offensive things
-thinks hes superior, degrades wife and inlaws
-minds stuff, oversensitive
-very confusing impulsive behavior
-forgetful at times
-throat clearing habit sucking through nose
-feels tired
-hypnogogic jerks/tics while sleeping
-has acne on face
-belly is coming out
-bad mouth odor
-varicocele with bilateral hydrocele
-low sperm count
-MRI was normal
-used to pull his small beard hair.
-eats junk food or eats a lot at times.
-father has ocd .
-procrastinates a lot

Upon your advice, he took Nux vomica 200 and Lycopodium200 but it didnt help. He said his pain increased.

Has the following with him right now but has not started(ordered incase condition due to heart issue): reckeweg R2, R3 heart drop, Crataegus, Aconite. Should he take any of these?

Thinking of ordering some of these Lachesis200, Hyoscyamus, pulsatilla, nat mur, calc carb, arnica but not sure which ones will be more helpful right now. Please guide. Thank you
