Trigeminal neuralgia - left side pain
HiI am 52/M/IND suffering from Trigeminal Neuralgia for last5 years.
Types of Symptoms : Initially pain started like electric tick for fraction of seconds while applying water soap to face. After four days electric ticks are converted to shocks from seconds to minutes. Shocks are started from left lower jaw to brain covering forehead, eye area of left side of the face. Severe pain was aggravating after Talking, laughing , Washing face, Brushing teeth, bathing.
I am taking allopathic drug Tegretol 400mg. But sometimes I need to take 1200 mg.
Also started taking Homeopathic Drug for last 1 and 1/2 month
1) Spigelia 200 C (15 - 15 Drops Morning and evening in water)
2) colocynthis 30 C (15 - 15 Drops Morning and evening in water)
3) Hypericum perforatum 30 C (15 - 15 Drops Morning and evening in
water)
4) China 30 (15 - 15 Drops Morning and evening in water)
Pain is reduced but not completely disappeared.
Please suggest me the correct drugs.

Do you remember any particular reason? Like injury, cold wave etc.
Left ear was operated due to hole in curtain and repeated pus formation approx in 2017.
Is it worse or better by applying cold/hot?
It is Worse by applying cold
Better or worse by pressure or rubbing?
It is worse by rubbing
Better or worse by movement?
It is better by movement but pain is on the pick it is worse.
Better or worse by chewing?
It is worse by chewing. But if I take Tegretol it is ok.
Is it worse at a fixed time?
No, it comes any time

Pain is worse in month of October every year
MRI done 6/12/2022 below are the details
MRI BRAIN ( PLAIN AND CONTRAST) Plain MR imaging has been performed. Heavily T2-weighted DRIVE images were acquired for evaluation of the cisternal segment of the trigeminal nerves and blood vessels / vascular loops in relation to the trigeminal nerves. No e/o acute infarct/fresh bleed /focal lesion is seen in the pons. The FLAIR images do not reveal any altered signal along the root exit zone of the trigeminal nerves. A Loop of left superior cerebellar artery is seen to cause splaying of motor and sensory nerve roots of left trigeminal nerve. It is looping around the root exit zone of the Left trigeminal nerve. Root exit zone of left Trigeminal nerve is sandwiched between left SCA and venous loop coursing along lateral aspect of the REZ in craniocaudal direction. A small venous loop is seen coursing along lateral aspect of REZ of the right trigeminal nerve on the DRIVE images. Normal appearance of the Meckels cave on either side. No obvious mass lesion or focal thickening along the course of trigeminal nerve and its branches is seen on present study. Visualized brain parenchyma appears unremarkable. Corpus striatum shows normal appearances. No posterior fossa lesion is apparent.
Normal ventricular system and basal cisterns. Flow voids of basal cerebral vessels look normal. IMPRESSION
A Loop of left superior cerebellar artery causes splaying of motor and sensory nerve roots of left trigeminal nerve and loops around the root exit zone. A venous loop is coursing lateral to REZ in craniocaudal direction. As a result, Root exit zone of left Trigeminal nerve is sandwiched between left SCA medially and a venous loop laterally. Possibility of neurovascular conflict is raised. No evidence of any focal mass lesion or abnormal signal/thickening is noted along the course of the trigeminal nerve and its branches on either side.

Stop all other medicines and start below remedies:
Arnica Montana 200C (2 drops in some water, twice daily)
Hypericum Perforatum 30C (2 drops in some water, thrice daily)
Morning dose, as soon as you get up, before brushing your tooth.
After noon dose: 2 hours after your lunch, make sure that nothing has been taken for 30 minutes before taking the medicines.
Night dose: before sleep, make sure that nothing has been taken for 30 minutes before taking the medicines.
Please maintain 30 minutes gap between these 2 medicines.
Keep updating me regularly.
Arnica Montana 200C (2 drops in some water, twice daily)
Hypericum Perforatum 30C (2 drops in some water, thrice daily)

I think u should take Tellurium 30c thrice a day. alongwith Arnica 200 one dose daily. 5 drops of each in some water. For atleast 1 week.

I started Arnica Montana 200 and Hypericum Perforatum 30C. But my observation my Pain little increased, is this a good sign in Homeopathy? Medicine is selected correctly.

Yes, It is improved now, but pain is not completely gone. How long it is safe to take homeopathic meds? Also there is headache whole day. some time throbbing headache.

No charge in headache. It is whole day. Yesterday night TN shocks also increase. Everything become unpredictable. Some time it is calm, sometime it is worse. How long it takes to effect homeopathic meds generally in TN

you can take Tellurium 30c. If it is ear related then it can work. But stop it if it does not work in 3 days

drsajid Sir received both meds and started 3 days back no changes in headache. Today tooth are active due to TN, also recurring shocks on left side of lower jaw and on forehead due to TN. It is impossible to slip on left side. when face touches to pellow shocks generate.

Can u take a single dose of Medorrhinum 200c,
As it is related to sycotic miasam which is often associated with chronic infections inflammation and nerve damage, it may help address the underlying constitutional imbalance and overcome the hurdle,
Then after 2 days u again take Spigelia alone.
