My son masturbates on furniture

My son tuned 4 years in Dec. I found out lately that he lays on the furniture and rubs himself on it when I see him he smiles. Also he likes to touch his genitals and be naked.
His urine smell strong like ammonia and he has tantrums sometimes and cry out loud then after a while relax and starts smiling like nothing happened.

When I was pregnant I was going through emotional crisis. He was induced and had jaundice after birth and was Colicky at 2 weeks old. Constant crying wakes up every hour to suckle on my breast.. didnt get much sleep when he was a baby. Cry all day all night
He used to run tantrums if things dont go his way. He can stay an hour angry. Hit and throw things
He wanted me to carry him at all time
He started to say words when he was 3.5 years old. Crawled and walked early. Teeth at 9months old without pain

Now he speaks but not clear I feel he is a bit behind on the intellectual level.
He stopped coming to my bed at night just few months ago. Now he come to my bed every few nights. He like to clinge to me
He has frequent colds and usually it goes to chest with right ear infection
Last time I noticed that he gets the right tonsil swollen while having the right ear infection that sometimes goes to left side. I also noticed a blister on the right tonsil.
He had first flu when he was 2 month old and it took a while to heal after it got worse from ear and chest infection. He had it again at 5 months old.

He is hyperactive and have low concentration and forgets a lot
He likes to eat snow, pickles like cold milk. Dont like meat


He is slim, narrow chest when he cries always ask me to wipe his tears
Like to walk barefeet in the daycare
Laugh a lot loudly when he is happy. He is cheerful and wants to make everyone laughs.
He used to sleep in the foetus position since he was a month old because he was Colic and it continued for 2 years now he sleeps on back with arm above head

Please give me advise I was detached from him from birth and wasnt connected but I managed to breastfeed him until 2.
