Occasional Stomach pain in 73 year old woman

My mom gets gripping pain like a trapped gas located just below her sternum. It is intermittent- sometimes twice in a week and sometimes not for two months. X-rays and ultrasounds show nothing wrong. The pain comes in early evening (7pm-10pm) and lasts 12 hours. Sometimes if she takes apple cider vinegar it is significantly shortened- to 4 hours. It doesn’t matter what she eats. If she passes gas it seems to relieve it a little bit. It is not related to constipation (she takes restorolax sometimes for constipation). She also has a hiatus hernia but that has been under control causing no symptoms for over 10 years using rabeprazole. What would you recommend for this strange stomach pain? Her other issue is that the tips of her nails are always lifting off of the skin, it may or may not be related. (So the nail itself is not splitting, but the nail is parting from the underlying skin and applying nail polish seems to temporarily help). She has been taking biotin for 2 months but it has not helped her nails. Thanks for your advice.
