Multi Symptom Issues

Hello I hope all is well with everyone here. All the best to everyone for this new year! To all better health and wellness within this helpful community.

About two years ago my brother (33 years old) began experiencing a sharp tearing pulling pain inside of his scrotum. He had told me that his inner hip (on both sides especially the left side) had become very sore and that he gets up to urinate frequently at night every night since with painful urination that sometimes smell pungent. He also mentioned that the veins connected to his testicles sometimes become inflamed and that both testicles become inflamed from time to time, sometimes at the same time and sometimes turn different sizes at different times. They are sensitive to touch at different hours of the day and night and there also appears to be swirls of particles in the urine which he mentioned are usually various colors when he urinates. He is troubled and has shared his concerned also because he has stated that his scrotum swells with fluid and is hot to touch and he has tried D-ammose, cranberry drinks and supplements and has given urine samples ay the clinic that came back negative for UTI and doctor told him that she did not find any sign of UTI. Please HELP as my brother hasnt been able to conceive any children and he doesnt believe in modern medicine as doctors have not been helpful to him. Professional advice (is preferred)is much needed and would be very much appreciated. Thank you all and a wonderful day. I tried to be as transparent and thorough in my description as I could be.
