Need help with insomnia. Please help

Hi

I'm not sure if I need to identify someone first. I'm not quite sure how to do that. If this isn't the right procedure, can you tell me how to identify someone?

Age: 52

Please answer the following questions in a descriptive manner after careful analysis and recollection of previous experiences and happenings.

1. Describe your main suffering?

2 chronic issues (may be related).

Insomnia - I have a really hard time falling asleep. I may start to fall asleep (actual sensation of falling) but then wake up in just a few minutes. Beyond this I get very sleepy around 4-6pm as if that is when my body wants to go to sleep, but even that will turn out to be a nap. Once I get to sleep, I'm good, but the getting to sleep is a problem. I have had sleep issues to one extent or another most of my life. For more than 2 decades I worked off-shifts because if I was going to be awake I wanted to get paid for it. Over the last decade I've worked very hard to have good sleep hygiene. No TV in bedroom, no electronics except white noise or brain entrainment device. No other lights, blackout curtains. Routine time to bed and getting up, even on weekends… Melatonin doesn't seem to help. Tryptophan is hit or miss. SSRIs are not effective. GABA based prescriptions are better but still not really consistent. I've tried something called Neurexan (a homeopathic). It can work if my mind is racing but most of the time I can have a calm mind and still be awake. Sometimes gently massaging my husband will work better than either of those. Again, not always but more often.

Note that there is a side/tangental issue to this in that sometimes my chest aches. I've had doctors confirm that it is not my heart or my lungs. It is right at my sternum. The ache is often in the evening and I've always associated it with not having enough sleep or enough good quality sleep.

Very tight muscles -
Second issue is chronic muscle tightness, especially in my Achilles tendon, calf, and my IT bands (illio-tibia?) on the side of the leg. Applying magnesium works a bit but not enough. I took a psychiatric med a decade ago that was addressed for depression in my case, but was originally used for tonic/clonic seizures. A side effect was muscle tightness. I discontinued the med, but the tightness has never gone away. I have inserts in my shoes which do a lot of compensation, but I still have tight muscle issues. I have to stretch almost daily just to keep limber enough to touch my toes.


2. What other physical sufferings do you have in your body?

Allergies.

Also, I have been diagnosed with Hashimoto's/Graves disease. Officially I am borderline. My general thyroid readings are normal but the sTsh is low. I've tried taking the meds to reduce the thyroid and they caused migraines, so I stopped that. I have refused to allow anyone to irradiate my thyroid because I do not want to be dependent upon medication for the rest of my life. I suspect this is an autoimmune situation.


3. What mental sufferings / feelings do you have associated with your physical sufferings?

Mentally I'm frustrated that I'm not finding solutions to these things.


4. What exactly do you feel when you are at your worst?

Wired and tired for the insomnia. Sharp pains on the outside of my right knew (more often than the left) and my right hip for the muscle tightness.


5. When did it all start? Can you connect it to any past event or disease?

Insomnia has been that way since childhood. See above for muscle.

6. Which time of the day you are worst?

Insomnia, if I get sleepy in the evenings it is around 4-6pm but I will get a second wind after that point. I mellow out around 9pm and go to bed at 11pm. Then the struggle starts for sleep.

Muscle tightness is after I have been sitting for a while. I have a sit/stand desk at work where I can sit or stand as is best for me. I also have issues if I try to lay on my side too much when preparing for sleep.

7. What are the things which aggravate your suffering and which are those which ameliorate the same?

I think I've answered these above

8. Do your think your sufferings have relation to any external stimuli (like, change of place) or any internal biological changes in the body, like, menses (in females)?

FYI - I am perimenopause but both of these issues pre-dated that.

9. When do you feel better, during hot weather or cold weather, humid or dry weather?

Before a few months ago, I would have said warm/hot weather, but my body is changing (menopause) and I seem to feel warmer now and less bothered by the cold.

10. Describe your general mental set up? Are you Moody, Arrogant, Mild, Agreeable Changeable, Nervous, Suspicious, Easily offended, Quiet, Arguing, Irritating, Lazy etc.

I am a bit driven. My mind is very alert and somewhat vigilant. I am fairly easy to get along with, but I do not take direct orders too well. I need to be involved in decision making.

- How do you feel before or during a thunderstorm?

No difference in general. It has been very wet over the last month (record rain) and mold allergies are kicking in. Someone at work told me that mold allergies are worst the week after the rain stops and that fits my experience.

- Do you like being consoled during your tough times?

Yes

- Are you sensitive to external stimuli like smell, noise, light etc?

Yes, noise and light and touch.

- Do you have any typical habit or gesture like nail biting, causeless
weeping, talking to one self etc?

Not that I am aware of.

- How do you feel about your friends, family, your children and especially your husband / wife?

My husband is wonderful; very caring. I have few close friends. I feel ok about most of my friends. Not sure what to say here.

11. What are your fears and do you dream of any situation repeatedly?

General fears are about being in no-win situations. I do not have any repeating dreams.

12. What do you crave for in food items and what are your aversions?

Crave chocolate. Other than that, cravings vary. Sometimes I crave red meat. Other times I crave fats (like sour cream or butter).

13. How is your thirst: Less, Normal or Excessive?

I stay well hydrated. I don't have a lot of thirst. I prefer cold water.

14. How if your hunger: Less, Normal or Excessive?

Hunger depends on how well I sleep. If the sleep is poor, I'm far more hungry.

15. Is there any kind of food which your body can’t stand?

not really

16. Is your sweat normal or less or more? Where does it sweat more: Head, Trunk or Limbs?

I sweat is about normal. Mostly it is on the back of my neck and upper back.

17. How is your bowel movement and stool type?

Fairly decent. Sometimes slightly more towards constipation but I think part of that is the tryptophan.

18. How well do you sleep? Do you have a particular posture of sleeping?

Sleep is poor. Mostly I sleep on my back.

19. Do you think you are able to satisfy your sexual desires in general? Yes

20. How do you think you are different from others, if at all? I have a sharper mind. I have less physical capabilities (poor coordination, balance, exercise stamina)


21. What medications have been taken earlier by you to treat the diseases and do you have any particular symptom surfacing after the medication?

I've taken most OTC sleeping pills and a good portion of the prescription pills for sleep and some for depression, anxiety. As noted above items that work on the GABA pathways are more effective than serotonin based meds.


22. What major diseases are running in your family?

My mother had multiple auto-immune issues. My family has a higher than chance occurrence of aneurisms. There is a statistically higher rate of cyclothemia/bipolar like issues in my family, but the psychological symptoms look a lot like the wired-ness of Graves, so I'm not sure of the source.

23. Describe, how do you look like? Describe your overall appearance

I am short and a bit stocky. Round face. Fair-Gaelic complexion.

24. (ONLY FOR FEMALES)

Please answer the following questions:
(Please give details of your past menstruation if you have attained menopause.)

Last period was a couple of months ago. Not consistent right now.

- Are the periods early, regular or late in general? How long do they last?

Most of my life my periods were long (up to 42 days) unless I was taking hormones. Usually short periods (~4 days). At one point (more than a decade ago now) I did have estrogen dominance.

- Do you suffer from any kind of physical or mental discomfort before, during or after the periods?

Some cramps. Painful breasts.

- Is the flow scanty, normal or excessive?

2 days normal, 2 days light

- Is the blood thick bright red or pale watery? Bright red

- Do you notice any clots in the flow? days 3-4 yes

Mental Status

ANS. I have a very good quality of life with my husband and friends. My work-life varies a bit. It is a challenging job and there are times when it seems there is no way to accomplish what is being asked. Some days (like today), I can remain centered at times others would call stressful. But there are other days when I get frustrated and feel cornered.

b)Any mental/emotional shocks occurring in the patient's life-grief, major financial losses separation from loved ones, death, identity crisis and other stress in life.
ANS. Nothing occurring currently. I had one situation recently where I felt my job was at risk, but that has been mostly resolved. Overall, this is the least stress period I can remember in my life.


c)Memory,ability to concentrate/comprehend.
ANS. Depends on the day. When I don't sleep well I can't keep up with the details. Most days I'm really good at following and applying logic.


d)Are you fearful of anything eg: Animals, people, being alone, darkness, death, disease, robbers, thunder, storm, high places.
ANS. Yes, I was abused in the past and things that remind me of that I am fearful of.


e)Are you anxious about anything: if yes, give details.
ANS. At the moment, nothing. As noted above this is really the least stress I've had in ages.


f)Are you impatient.
ANS. Sometimes, when I am tired - not enough sleep.


g)Are you doubtful or suspicious.
ANS. No.


h)Are you hurt easily (emotionally)how do you react. Does it cause hatred/revenge.
ANS. I am more sensitive than average. I tend to show my feelings more than most. I don't believe in hiding a part of myself. If I am in a safe environment/people I will be sad. If I am not safe - I tend to be more angry. However, I don't tend to hold hatred or want revenge.


i)Does your pride get hurt easily.
ANS. No. I have one person who can be critical and it hurts if she treats me with contempt, but normally she doesn't do that anymore.


j)Are you depressed, if so, reason/circumstances.
ANS. Currently no. I have been in the past but not now.


k)Do you like to share your problems.
ANS. Yes with my husband. Not with many people though.


l)Effect of consolation.
ANS. Touch calms me, but words and empathy don't do much.


m)Do you ever become suicidal when? How.
ANS. When I was young, I was depressed for a long time. My best friend had been killed by a drunk driver and no one wanted to discuss or acknowledge her death.


n)Memory- quality if poor, for what ( eg. Names, places, people, what you read).
ANS. I find it hard sometimes to find the right word. I also find it hard to remember long lists of things to do.


o)Do you weep easily, effect of weeping, ie, does it make you worse or better.
ANS. Sometimes yes. When I saw the animals in the flood in Tibilisi it was sad. I don't think the crying makes a difference.


p)Are you easily irritated. What makes you angry, how do you express it.
ANS. When I'm irritated I may interrupt people and not listen. If I get very angry I get quiet.


q)Are you destructive.
ANS. I have been in the past, but not now.


r)How good are you in making decisions.
ANS. Very good.

s)Do you like company or like to remain alone.
ANS. It depends on how tired I am and how much I've been around people at work. I don't like either extreme.


t)How seriously are you affected by disorder and uncleanness in your surroundings.
ANS. I am not seriously affected until it impacts my ability to accomplish something.


u)How does failure appear to you?
ANS. Not living up to my own beliefs and expectations.


v)Are there any matters that you deeply dislike?
ANS. I can't think of any.



w)What activities you deeply like? How does it affect your mood?
ANS. I love reading and debating. When I'm with other people, even if we disagree, a debate is fun and I feel energized and happy. But if it goes on too long to the point I'm tired, it weighs me down and I get irritated.


x)Are you affectionate? How does others sorrow affect you?
ANS. I am very affectionate with those close to me only. Another's sorrow will affect me differently depending on whether I am empathizing or trying to be strong for them. If I empathize, I feel much of what they feel. If I'm trying to be strong, I don't feel as much.


y)Any present fears in your life or future.
ANS. No


z)Any present life or future life desires.
ANS. I want to finish remodeling my house.

Tongue - no meaningful taste. I have mostly a pink tongue, but it is a bit lighter in the center than on the edges.
Facial - I have a very red face as I have rosacea, every once in a while (not frequently) I may have a pimple

take COCCULUS INDICUS 200c liquid, 2 drops in a tablespoon water, only 2 dose not more than that, not daily, 1st dose before sleep and next dose next morning after wakeup,

{if buying pills then 3 pills as one dose, 2 times, 1st at night and 2nd after wakeup, chew it, do not swallow with water}

do not eat or drink anything 30 minutes before and after medicine,
