IBS. Need help from dr. kadwa

Okay, this is the history. Question: after taking nux vomica 2 weeks ago, some improvement but not normal as yet. Daily, 1 out of 3 times, normal motion. Cold okay, itchy eyes continue, partial constipation, wanting to go, sometimes not, incomplete etc, continues. What take now? back to nat sulph or nux vomica again?

1. Describe your main suffering?
- IBS with diarrhea and partial constipation like symptoms. Soft stools was chronic for more than a year until recently. Have lost 4 to 5 kilos in the last 5 years. Stabilized now. This was seldom accompanied by an ache. But with fullness, especially late afternoons. Partial constipation: never a complete motion. Need to go three times a day, mostly mornings, once afternoons; small stools frequently. Some gas occasionally. When react to certain foods, only then very loose stools.
Discovered h. pylori in endoscopy last year, treated with allopathy, no change.
2. What other physical sufferings do you have in your body?
Minor ailments: dry, itchy eyes: always when have a cold with slight sneezing. But even without a cold, especially in dry weather, it continues.
Moderate level osteoporosis: Coincided with ISB. Ache in one finger, back (in control now) and ache during a certain posture down the right side of the right leg.
Common cold once a year; occasional mild cold with sneezing, some white sputum from the throat, infrequent headache accompanying it – only when sneezing is too much. A dry cough used to be common (maybe due to exposure to pollution then) but not since a few years.

3. What mental sufferings / feelings do you have associated with your physical sufferings?
None since all are mild and energy levels seems to be almost normal. Also don’t get involved too much in bodily suffering unless extreme.

4. What exactly do you feel when you are at your worst?
IBS: When at my worst it is usually loose stools for a day or so accompanied by some pain.
Osteoporosis: when exert too much, back ache.

5. When did it all start? Can you connect it to any past event or disease?
Used to have loose motions all of my life once a year or so. May have started with the use of antibiotics. Became chronic 3 years ago. Had stopped the use of antibiotics since for infection so let it continue and tried various remedies.

6. Which time of the day you are worst?
No specific time. Could be mornings if bm is not complete, could be afternoon if fullness persists. Evenings are always the best. By 6.30 any discomfort settles down and I can eat well.

7. What are the things which aggravate your suffering and which are those which ameliorate the same?

Sensitive to unhygienic food, especially street food, occasionally at restaurants as well. Maybe sensitive to certain foods, but don’t really know what exactly. Still trying it out – maybe peas, cabbage, spinach, cannot overeat anymore. Maybe be sensitive to non-vegetarian food, needs to be tested once more.
Eating less feels good.

8. Do your think your sufferings have relation to any external stimuli (like, change of place) or any internal biological changes in the body, like, menses (in females)?
Lead a very routine life. Doesn’t flare up during menses. Change of food, especially if eat at a restaurant, YES. Could also be if I eat home cooked food at another’s home.

9. When do you feel better, during hot weather or cold weather, humid or dry weather?
Like hot weather. Dislike cold, rainy weather. But symptoms about the same. But the annual flare used to happen usually just post monsoon. Can’t stand air conditioning.

10. Describe your general mental set up? Are you Moody, Arrogant, Mild, Agreeable Changeable, Nervous, Suspicious, Easily offended, Quiet, Arguing, Irritating, Lazy etc.
Usually mild tempered, quiet, not a conversationalist, introvert, solitary (but can appear to be very friendly and can even talk when the situation calls for it). Usually cheerful. Like routine, not stressed out for the last 5 years since quitting full time job. Previously there was stress. Not since. Not easily offended but get offended by superious attitudes and hence show some irritability sometimes. Anger – infrequent. Not argumentative usually. Down to earth – like walking, taking public transport etc. Tend to be simple and austere. Not lazy, very hardworking at home and at current place of work (part time NGO) but love having no activity to do. Not laziness but love for still life and still mind. Used to go after my hobbies with zest and passion to the extreme (but never so in working life). Quite contradictory in many ways: lazy (enjoy silence and non-doing) and hardworking, passion for one or two things and dispassion for most things; come across as friendly and quiet but can be impersonal and talkative. Come across as polite and nice but can be rude and undiplomatic …. Obsessed with music for decades but out now. Love reading. Used to love travel, not any more. Skeptical about romance and relationships. Keep a distance always. Impersonal friendliness is fine.
Do yoga and some walking these days for osteoporosis but between age 20 and 40 very little physical exercise.
- How do you feel before or during a thunderstorm?
Used to fear thunderstorms as a child, but change in the last few years. Not enjoyable but tolerate it quite well and see the beauty of it too.
- Do you like being consoled during your tough times?
No. don’t share inner feelings with anybody. Work it out.
- Are you sensitive to external stimuli like smell, noise, light etc?
Not usually. Can get behind the noise even during firecrackers.
- Do you have any typical habit or gesture like nail biting, causeless
weeping, talking to one self etc?
No.
- How do you feel about your friends, family, your children and especially your husband / wife?
Unmarried, live with old parents. Get along well with parents and visiting brother, do my duties, cheerfully.
11. What are your fears and do you dream of any situation repeatedly?
Not really. Inconsequential dreams. Don’t remember dreams frequently.
12. What do you crave for in food items and what are your aversions?
Don’t ‘crave’ for anything. Can live without most things. But I love good coffee (don’t drink it much anymore), used to love bread and butter and cheese (bread/butter has been cut out of the diet but cheese is still on), used to love pasta, fish, but no more. Prefer rice to rotis (cut out wheat rotis now on jowar or corn), love the south Indian sambar, coconut preparations, idlis and the like. Not crazy about north Indian food. Love lime and lemony to sweet. Almost cut out non-vegetarian food. Eat a lot of yoghurt for osteoporosis but don’t necessarily love it. Like all vegetables. Like fruits but not crazy about them. Eat them daily though. Like hot drinks. Used to love wine, but off alcohol now (didn’t drink much ever though). Never smoked.
Aversions: none really.
13. How is your thirst: Less, Normal or Excessive?
Less. Usually never feel thirsty unless exertion and hot. Force myself to drink 5 glasses a day.
14. How is your hunger: Less, Normal or Excessive?
Normal although quantities have reduced. Mornings not too good. Lunch a little better. Afternoons, can eat a snack now when feel normal. After 6.30 ready for a good dinner. It gets better through the day, except late afternoons when I can’t put down much. But mornings need to be light.
15. Is there any kind of food which your body can’t stand?
Testing…. Non-veg gave me a problem twice but not sure it was that. Could have been something else. But never reacted adversely until recently. Spinach maybe, peas possibly.
Can’t take cold drinks or ice-cream. Affects the throat – leads to sneezing and a cold.
16. Is your sweat normal or less or more? Where does it sweat more: Head, Trunk or Limbs?
Less to normal. Mostly the trunk.
17. How is your bowel movement and stool type?
Used to be semi-soft for over a year, dark, need to visit three times a day for complete release. When react adversely to food, becomes smelly and some gas. Recently normalized.
18. How well do you sleep? Do you have a particular posture of sleeping?
8 hours. Usually through the night. Sometimes wake up and difficult to go back to sleep. But usually good sleep. Either right or left side. If the leg gets too painful which it does sometimes when one is atop the other (of late this has been happening) then on the stomach for a short while.
19. Do you think you are able to satisfy your sexual desires in general?
Done so and now no need.
20. How do you think you are different from others, if at all?
Change in the last 3 to 5 years. Opted out of stressful managerial work life to a semi-retired life. happy with passive home work and some work at ngo. Ambitions, position, money, relationships, creative expressions, interests, etc. are not important anymore having done them all (secure financially). The only aim is to figure out the source of this life and bloody serious about this. Life now revolves around meditation (which may have produced some upheavals in the health in the last 2 years, can’t say) and trying to understand life in a global perspective rather than an individual one.

21. What medications have been taken earlier by you to treat the diseases and do you have any particular symptom surfacing after the medication?
- Used to take tinidazole and the like for loose motions. Stopped now, except early 2010 had to take it post H. Pylori discovery. Stools got back to normal for 5 days and then one meal out got it back to pre-pylori condition – semi stools again.
- Anti allergics although minimal usage (for the cold).
- calcium and calciferol (vitamin D3) for osteoporosis. Also tried calcitonin nasal spray for 3 months. It helped.
- Took the usual malaria pills 6 months back for malaria.
- Had severe paratyphoid 20 years back. Took a whole lot of antibiotics intravenous and developed an allergy to penicillin then.
- Last year had 2 bald patches on head – alopecia areata. Took some allopathic ointments and vitamins for it. Okay now.

Tried homeopathy for a year (didn’t know what I was taking). Some symptoms like fullness, gas etc. cleared, stools got better from loose to semi but remained that way. Then 6 months ago, went on to abchomeopathy.com and was advised nat sulph 6x which turned around the stools to absolutely normal. Since then, taking it as and when required.
Also tried pulsatilla, nux vomica, rhus tox for various things but can’t say if they helped any conditions. Too ambiguous since took them a long time after prescribed (time to go and get the pills during which time symptoms either drop on their own or I resort to an anti-allergic or whatever). These still need to be tried. Usually my condition is so ambiguous, recovery too, life too is ambiguous that it gets difficult to pin point the exact cause of things.

22. What major diseases are running in your family?
Mom: mild osteoporosis, but a lot of back ache, knee ache, recent compression fracture of spine on fall, very sensitive skin, allergic to certain medicines, milk…
Dad: mild blood pressure under control

23. Describe, how do you look like? Describe your overall appearance
Age 45, female, five feet 4 inches, 46 to 47 kgs as of now, wheat complexioned but slightly on the fairer side, brown eyes, brown to black fairly straight hair, tend to look younger than age, slightly tomboyish in appearance. Mild demeanour, bad posture slightly hunched (although very confident and independent).

If you are not having normal menstrual cycles, please answer the following questions:

- Are the periods early, regular or late in general? How long do they last?
Earlier years, always late. Took some pills and got regulated. Last 15 years or more: regular 28 days more or less. Plus or minus 2 days depending on exertion. Now a days, sometimes, 2 6 days if been a physical active month.
- Do you suffer from any kind of physical or mental discomfort before, during or after the periods?
Stomach aches rare; usually one of two symptoms: snappy and irritable either just before or on day 1 OR a headache.
- Is the flow scanty, normal or excessive?
Last 5 days. 1 day – medium to heavy. Second day: light to medium. 3rd day onwards, light to very light.
- Is the blood thick bright red or pale watery?
Not too thick, not too pale, neither too bright.
- Do you notice any clots in the flow?
No

one more common issue to be added to the history - itchy throat frequently on having even slightly cool stuff. turns up at night.
