Strong will (she used to drink a lot of coffee but has now completely quit after being introduced to homeopathy. She has had no coffee this year. )

Sleep: Position - starts on the back but usually wakes up on the left, Left side.
Sleeps with feet uncovered Nicole says not because they are hot or anything but because she likes them unrestrained
Got her menses when she was 10 and a half years old.
Menses are regular and normal. Lasting about three days.
In the last 1 year or so she has pain in her coccyx that begins a couple of days before her period and is ameliorated as the period begins
Brief Personal History
Nicole has suffered from a lot of grief and violence.
Nicole was beaten up by an abusive Boyfriend at the age of 15, suffered from head injuries and even a skull fracture. This probably caused her migraines that started a few weeks after the skull fracture.
Nicoles parents and grand parents died in quick succession. Mother suddenly died of a blood clot and her father was stabbed.
Elder sister, whom she loved more than anyone, died in a car crash.
Nicole is now totally devoted to taking care of her elder sisters three sons.
About Four years ago while returning from work at night she was attacked in the street.
This man tried to rape her but she fought back and escaped. Ever since then she has been sleeping with her TV on. She says she has trouble sleeping in silence and needs the TV noise to sleep.
Two major medical problems have been Migraine and chronic bronchitis.
Migraine was probably a result of trauma to the head.
Chronic bronchitis, about 10 years ago, was brought about as she was exposed to coal fumes from a coal furnace in the same room while sleeping. She woke up with a cold and it soon turned into a severe bronchitis, she felt like her chest was on fire, for which she received allopathic treatment, but ever since then she has a cough on and off that seems to bother her more in hot humid weather. This cough now can be dry or with mucus but hardly has that burning sensation.
Brief chronology over the last 3 months
She took Sanguinaria 200C for her migraine on 21 June 2008. Her symptoms for migraine were consistent with head symptoms of Sanguinaria: pain begins in occiput, spreads upwards, and settles usually over the right eye.
20 June 2008 migraine with nausea, backache in coccyx with her period on.
21 June 2008 Sanguinarea 200C at 7 am symptoms get really worse and she could hardly move her head and had to sit still through the agony for about 20 minutes, then the migraine began to ease and in another 30 minutes completely disappeared. Not only did the migraine disappear but also the backache associated with her period
23 June 2008 wakes up with pain in the left hip area after a thrashing sleep. Her girlfriend who was sleeping in the same room noticed that Nicole was tossing up and down as if thrashing the bed. Nicole woke up with no memory and only pain in the left hip. This pain lasted a week and disappeared.
A week or so later Nicole developed a cough that would come with a tickling sensation in her throat. This also lasted a week and went away.
Towards the last week of August, Nicole noticed that she was feeling more tired than usual, was constipated alternating with loose stool and noticed weeping moods. Then in the first week of September she had her first palpitation that made her dizzy and she had to sit down. This was followed by another similar episode
Second week of September Nicole was diagnosed with Hypothyroidism with a TSH of 72. Now she is on levothyroxine sodium. Her cholesterol is 302.
At the clinic when the Dr. told her TSH levels were critical Nicole had another episode of palpitations and fainted. This was in her memory the only time in her life she fainted
Current mental and physical state
Sad, depressed, anxiety about health, weeping and tearful moods. Overwhelming urge to cry. Cries alone. Breaks down on the phone with her boyfriend and apologizes saying I dont like this crying, this is just not me I dont know whats happening to me
Although normally there is more fluttering but any slightly bad news now will cause palpitations of the heart.
Fatigue, tired and wanting to go to sleep. More dreams than before.
Feels even more tired and sleepy after eating anything
Sleeps on the back and the left side. Starts on the back and wakes up on the left side.
Sleep is un-refreshing and disturbed.
Stools constipation alternating with loose stool. Lately however its more constipation probably from the levothyroxine sodium.
Clears the throat while talking. (This symptom has appeared in the last 6 or 7 months)
Feels her voice is hoarse than before.
Skin is dry and flakey.
Minor body aches all over, right knee and right shoulder near collarbone. (She had injured her right shoulder rotor while attempting to lift something heavy three years ago)
Tingling in her soles upon waking up that goes away as she starts walking.
Pain in both soles, as if beaten worse in the morning, worse on walking, standing and pressure. She has to sit with a pillow under her feet. This could be a side affect of the Levothyroxine as it started the next day.
Muscle cramps in calf and toes at night.
Blurry vision after reading for hours, have to rub eyes.
Sameer your opinion?
Can her present state be attributed to the aggravation she had from sanguinaria 200C?
Or did sanguinaria remove a layer and now a new layer has surfaced?
Should her condition now be seen as acute state or chronic and treated accordingly?
Which is the most likely remedy for her now? Constitutional or acute
Note
I introduced her to homeopathy about three years ago.
Since then she has taken the following medicines for injuries and minor complaints two or three times Arnica 200C , Hypericum 200, Ruta 200, rhus tox 200, bryonia 200, pulstatilla 200. Sanguinaria 200 just once.
Last remedy she took was Pulsatilla 200 2 pills diluted with half a glass of water a month ago. It didnt seem to have any effect.

Please give Nicole a single dose of Causticum 200c, and then report to me in 10 days.
Make sure nothing enters her mouth 1 hour before and 1 hour after the remedy.

Thank you for your response. I had shortlisted Calc carb, Nat Mur and Causticum. What do you think of Nat-m and calc ? what mostly made you go for caust ?
I also considered Ign, sep, puls, phos Nux-v.
I dont believe Nicole is proving sanguinaria but could the severe aggravation in her head have caused damage to the thyroid which is linked to the pituitary in the head ? Although I understand that the high TSH is only an effect of the thyroid getting sluggish and not the cause.
I found Irisan interesting read as it is the most similar to Sang in terms of the migraine that nicole had on the right side and also the pain in her left hip that she experienced post sang. Was her body indicating here that she needed Iris ? Further Iris is also in the repertory for thyroid problems. Although Iris does not have the totality for her symptoms , but what if sanguinaria was a mistake and thyroid problem is another indication towards Iris-V ?
I understand these questions have no easy answers and I hope I am making some sense here. Anyway, would like your thoughts on it.

The whole picture points to Causticum, starting from mental, with history of repeated griefs, to history of bad bronchitis ,as well as desire to clear the throat at all times.
Even thyroid getting sluggish is an effect of disease as the cause of disease is dynamic.
Please try Causticum 200c and get back to me. She should feel better

I know it is late night now but if you are up able to suggest something right now as Nicole is having what seems to be an acute crisis now with her head pain. its a very severe stabbing pain that feels like stabbing with an ice pick slightly to the behind and above of her left ear.

I am extremely sorry as I was away and could not could not log into the forum to apprise you of Nicole's condition and also that I could not go ahead with causticum 200C as advised by you. I made nicole read up on calcarea carb, Natrum mur, causticum and a few other remedies. we decided to try calcarea first as nicole felt she was more like calcarea than causticum and she had calc 30 with her.
let me again briefly bring you up to speed on her condition.
7 oct 08 - took calc carb 30C one pill at 3 pm. the immidiate reaction was that she got these empty eructations(burps) like one gets after a fizzy drink. about one burp every hour or so- Not much but according to her she never used to burp so its worth mentioning here.
8 oct 08 - passed stool for the first time in two weeks without the help of any any laxative, woke up with less pain in the soles.
09 oct 08 - no stool, but less pain ( the same as on 08 the oct ) burps continue, and energy level a little better
10 oct 08 - took calc carb 30 1 pill at 130PM, few more burps., no stool
11 oct 08 wakes up with severe pain in right knee, soles hurt more.no stool.
About the pain in her right knee - she had broken her right foot and injured her right knee in a skating accident in winter of 2004. ever since then she had been getting a little reminder of the pain in her right knee every month or so especially in warm humid conditions.
12 oct 08 - pain in right knee continues. pain in soles back to pre calc level.no stool
13 oct 08 - pain in right knee continues but is less. everything else remains the same.
14 oct 08 - pain in right knee disappears. pain on right side of the neck close to the collar bone appears. as mentioned in my first post she had injured her right shoulder in 2006 and this had also caused her pain in the neck close to the collarbone.no stool
15 oct 08 - pain in neck continues. she took a laxative. pain in the small of her back.
worth mentioning here that she felt better as far as energy her energy level is concerned. about 50% more energy in the last week as compared to the period before starting calc carb. But the past 3 days had also been very emotional for her as her dog of about 12 years was sick and died on the 15 ( today )
now despite getting less sleep and being worried about her dog she had felt more energy than before. could this be calc carb or that she had more adrenlin in her system because of the emotions?
16 oct 08 - pain in her neck eases. pain in the small of her back also gone.
17 oct 08 - pain in neck gone.
20 oct 08 - pain in the coccyx coming back. this she says is a sign of her period as its due on the 23rd.
22 oct 08 - took a laxative.
she got her period on the 24th and it lasted three days. It was normal in all respects except that the small of her back hurt more this time around.the pain in her coccyx begins to ease after the first day of her period and and completely goes away by the time her period is over.
we did not repeat calc carb 30 as we wanted to wait till everything associated with her period was out of the way.
27 oct 08 - took calc carb 30C at 3 PM 2 pills dissolved in 5 oz ( 150 ml ) water and one sip taken from it. she had felt her energy levels drop again in the last couple of days. In the first hour of her taking the remedy she got more flutters in her heart plus minor pains and aches all over the body, the burps again.
she has been getting these flutters almost every day for the last nearly 2 months while palpitaions come only like once a week especially when she is emotionaly aggravated.
30 oct 08 - she passed stool today with out any laxative. In the last 3 days since taking calc carb energy level is up again slighly and she has to clear her throat less often.
01 and 2 nov 08 - soles hurt less by about 20%.
03 nov 08 - felt a slight pain in close to her left year and on her left temple.
04 nov 08 - by evening pain on the left side of the head behind her ear is continous.
05 nov 08 - This is really an acute crises now. she couldnt go to sleep at all as the pain behind the left ear seems to be more and more like a very bad migraine.the pain is bringing her to tears and she had to take a pain killer. she describes this pain as a stabbing as with an ice pick every 5 or 10 seconds. no pain in her left temple though.she has also noticed a dull pain between her shoulder blades.
Sameer as you can see she has only taken three doses of Calc Carb 30C.
calc carb 30C certainly seems to have helped a little. But questions remain.
Is the present migraine like pain a dis-similar aggravation or a return of the old symptom even though its on the left side and NOT the right side as her case was when she took sang.
She just took Nux vom 30C to see if it helps her pain now.

You are repeating the same potency too much and first repetition was done too soon. Anyways, I do not think Calc is the indicated remedy.
Also, regarding my advice, the prescription of Causticum still stands, either as 3 doses of 30c, all to be taken on the same day or as a single 200c dose, and this is not to be repeated.

Thanks for the prompt response.
Nicole took one pill of Sepia 30C about an hour ago and she seems to be better now. the pain is is not every five seconds but every 5 mins. can she take causticum 30C before this acute crises passes or should she repeat sepia 30C ?

No repetition of Sepia is needed.
You can wait for this acute episode to pass, and then give 3 doses of Causticum 30c equally spaced by 1 hour and no further repetition.
Each dose = 1 teaspoon from 250 ml bottle of spring water where 2 pellets have been dissolved.
Report in 10 days after the 3 doses.

Will wait till this acute crisis is over.
Her feeting hurting less, her throat irritation less and no flutters for the past 24 hours indicate that this acute disease has taken over from the chronic ailment. This appears textbook but the realtionship of this pain in the head is hard to understand. Is this a part of a natural healing crisis, an accessory of calc or a dis-similar aggravation ? or is this too soon to tell maybe time will provide an answer. I would certainly appreciate your thoughts on this sameer.

The pain in nicole's head has just come back with the same intensity. should she repeat sepia or nux vom ?
Also on 6 nov 530pm her time she had developed an acute pain in her right arm. this pain was gone when she woke up today. the pain was needle like and in a spot between her elbow and the shoulder, accompanied by tingling in her right hand on the palm and back of the hand. this pain prevented her from being able to lift her right arm up. as mentioned earlier she had hurt her right shoulder rotator cuff in 2006.
the tingling in her hand could be related to more than a year ago when she had been bitten by her own dog in the hand and she had taken hypericum 200C then. the pain then had completely gone away but she had experienced a jumping or an involuntary movement of that wrist every month or so since then
how do we deal with the excruciating pain in her head now ?
I have asked her to take a dose of sepia 30. she will take it at about noon gmt. please let me know if you have any other ideas as this pain is killing her.

No. we have not tried causticum yet.
