swelling and extreme pain in tubules in scrotum

Problems

1: Swelling in all ducts in scrotum and it pains very badly.

2: Prostatic fluid discharge on just thinking about sex or that causes pain in genitical duct and in testicles. In scrotum veins swell and starts pain untill the sperm ejected through masturbation.

3: in hurry starts orgasms and discharge occurs.

4: nightfall mostly.
5: masturbation once a weak, not by desire but to stop swelling and pain in lower pelvic region.
5: inflamatio in back side of left shoulder after discharge.

I am unmarried.
Semen discharge during urination and after urination. At the end of urination I feel, there left some urine but when I discharge actually it is semen.
Weakness, looks as I am 60 years old.

History
Masturbation at age of 13 and still doing at 28 age because of pain and swelling in scrotum.


Mental sufferings / feelings
Sadness.

Hiding trend.

Wisdom and intellects are effected and weakening.
Weak memory forgets words while talking, cannot recall and find proper words.

Hurrying in everything like eating, walking, talking, working etc.

Buried in thoughts. Continuous thinking without control

Shyness, feel extreme shyness if enter in a gathering where people were already present. Feels a kind of shame and inconfidense in talking in gathering and public speaking.
No love & affection for others except children.

Semen and prostatic discharge since 1 years.

I am extremely worried about. Please suggest a better way.
