Chronic Left Mastoiditis Case

Enlarged left mastoid for last 14 months.

Recently tried the Sil, Hep, Bell combo advised on recent mastoid post from Dr. Kadwa in Jan which helped more than anything else I've tried. I continued it for the 5 days as advised. Have been waiting for several days but no continued improvement and ear fullness is coming back.

Any advise or information would be greatly appreciated.

Please answer the following questions in a descriptive manner after careful analysis and recollection of previous experiences and happenings.
1. Describe your main suffering?
Chronic left mastoiditis lingering after severe ear glue and infection. Original infection likely started 24 months ago, with enlarged mastoid present for last 14 months. Reoccurring infections for seasonal afflictions (colds/pollen).

2. What other physical sufferings do you have in your body?
Moderate hip and lower back pain. Left shoulder pain with stiff scalene muscles.

3. What mental sufferings / feelings do you have associated with your physical sufferings?
Increased anxieties over own health.

4. What exactly do you feel when you are at your worst?
Pain (pressure) in left mastoid moving back across the temporal skull bone. Fullness in the ear.

5. When did it all start? Can you connect it to any past event or disease?
Cold with ear ache in Feb 2016.

6. Which time of the day you are worst?
More inflammation / pressure at night. Stuffy ear/nose (left) in morning.

7. What are the things which aggravate your suffering and which are those which ameliorate the same?
Aggravation from corn, which is a new food allergy to have developed during this ailment. Heat packs help the ear feel better.

8. Do your think your sufferings have relation to any external stimuli (like, change of place) or any internal biological changes in the body, like, menses (in females)?
Possibly increased stress from work if any.

9. When do you feel better, during hot weather or cold weather, humid or dry weather?
More on the hot weather or warm rooms.

10. Describe your general mental set up? Are you Moody, Arrogant, Mild, Agreeable Changeable, Nervous, Suspicious, Easily offended, Quiet, Arguing, Irritating, Lazy etc.
Calm, patient, quiet, nervous (sweating a lot) – fight response, anxieties from larger social settings

- How do you feel before or during a thunderstorm?
More apprehensive before the storm
- Do you like being consoled during your tough times?
Yes
- Are you sensitive to external stimuli like smell, noise, light etc?
Unexpected loud noises
- Do you have any typical habit or gesture like nail biting, causeless weeping, talking to one self etc?
Talking (thinking) out loud possibly
- How do you feel about your friends, family, your children and especially your husband / wife?
Deep concern for emotional wellbeing

11. What are your fears and do you dream of any situation repeatedly?
Fear judgment / examination in social encounters with new people. Dreams are of being chased or hiding.

12. What do you crave for in food items and what are your aversions?
Breads. Dislike milk, but can tolerate small amount in cereal.

13. How is your thirst: Less, Normal or Excessive?
Little to no thirst, intake is always low

14. How if your hunger: Less, Normal or Excessive?
Normal

15. Is there any kind of food which your body can’t stand?
Corn or corn products. Other extended family members have this allergy. Likely caused sinus infection in the first place.

16. Is your sweat normal or less or more? Where does it sweat more: Head, Trunk or Limbs?
More in morning. Arm pits.

17. How is your bowel movement and stool type?
Normal.

18. How well do you sleep? Do you have a particular posture of sleeping?
Toss and turn. Back or side.

19. Do you think you are able to satisfy your sexual desires in general?
Yes

20. How do you think you are different from others, if at all?
Highly sensitive / very empathetic

21. What medications have been taken earlier by you to treat the diseases and do you have any particular symptom surfacing after the medication?
Antibiotics – 3 rounds (pills & iv)
Kali Mur – helped ear glue
Merc Sol – didn’t do much, but relieved some pressure in the ear
Sil + Hep + Bell – This was taken from a previous post for chronic mastoid. Created a moderate headache, but the sil was very soothing to the mind. Continued 2 pill each, 3 time a day, for 5 days. This caused slight movement in the mastoid (slight improvement).

22. What diseases are running in your family?
High blood pressure. Elevated blood sugar. Sister had pancreatic C.

23. Describe, how do you look like? Describe your overall appearance.
6 foot, large build, overweight, light complexion.

15. Mental Status
a)The quality of the patient's life in relationship to loved ones, family, friends and colleagues. Overall quality of energy available to function in daily life, and under various circumstances.
ANS. Good. Limited energy – very sedentary
b)Any mental/emotional shocks occurring in the patient's life-grief, major financial losses separation from loved ones, death, identity crisis and other in life.
ANS. Loss of sister age 21, loss of wife age 34
c)Memory,ability to concentrate/comprehend.
ANS. Good memory. Difficult to concentrate – from anxiety/overwork
d)Are you fearful of anything eg: Animals, people, being alone, darkness, death, disease, robbers, thunder, storm, high places.
ANS. People – social judgment / examination
e)Are you anxious about anything: if yes, give details.
ANS. Too much work
f)Are you impatient.
ANS. No, very patient
g)Are you doubtful or suspicious.
ANS. No

ANS. No. Ignorance, usually not expressed

ANS. Fine with uncleanness as long as its my own – I know where it is.

ANS. I can only do so much

ANS. Injustice

ANS. Playing music, calming.

ANS. Yes. Deep empathy if I console

Thank you Dr Basu. I've completed the 3 days and have seen significant improvement in the swelling/inflammation behind the mastoid towards the back of the head. The mastoid area itself has become less inflamed. Great effect on the ear itself and also have improvement in the sinus. It seems evident that I still had an infection this whole time.

I think I still have a lot to drain out and understand that the mastoid can take some time once the infection is resolved. Is there any advice you can provide on possible need for a weekly "booster" dose if indicated or another continued treatment course? Otherwise I will continue to monitor and advise if symptoms worsen.

Significant gratitude to you in your aid in this matter. I am very thank for that you are helping us all with your skills.

Take Hepar Sulphur 200c for two consecutive day in following way:
Day1:Hepar Sulph 200C single Dose
Day2:Hepar Sulph 200C Single Dose
Day3 to Day7:No medicine
Repeat the cycle and feedback after two weeks

Dr Basu,

Pleased to report significant improvement over the last two weeks.

Upon completing the first weeks single dosing (day 2), I had some aggrevations. It could have just been some silimar aggrevations, but all of the symptoms matched past sicknicesses. Most of the symptoms were like a acute sinus infection (copious drainage, yellow. Sinus pressure. Sweaty. slight fever. Cough. sore throat, stitching to ear.). One peculiar symptom was increased groin pain. I’ve had this pain undiagnoised since age 16. Seem to match the Hep’s lymph pain – inguinal. Other peculiar symptoms was a small chest eruption/blemish.

Cold symptoms improved after a couple days with a lingering dry cough that failed to produce much except in the mornings. There was significant drainage in the mastoid area and complete drainage behind the mastoid to the back of the head (temporal bone) over this week.

Additional week’s dosages helped reduce the cough and also helped continue the mastoid drainage. Several pimple like eruptions around the mastoid behind the ear. Only returning symptom, after about 4 days of last dose, is like something being lodged in the ear. Waiting and working the eustachian tube to drain the ear has helped that some.

The mastoid swelling is down almost to when I first noticed it over a year ago. I would venture to say possibly a 75% improvement with this. Please advise if it is permissible to continue treatment?

Dr Basu,

Reporting continued improvement with some reduction in the size of the mastoid. The feeling of returning symptoms in the ear/throat are taking longer to come back between doses, and now seem to be reducing in intensity. It seems to be at about a full 7 days before feeling anything verses the 4 or 5 days weeks back. Feeling better overall. Good sinus drainage from the ear for most of this last week.

Is it permissible to continue in this treatment?
I'm tempted to ask if I just need to start tapering out (adding length between dosages) based on symptom re-occurrence, but I don't want to overstep my bounds. I worry consuming your time as I fear it may take several more weeks to drain this out.

I can not thank you enough for help and your time. I feel bad for taking up so much of it.

Hope you are well. No significant physical improvement noticed, but have been under high stress at work the last few weeks. I fear this has injured/slowed my recovery progress. I've had some changes today in assignments to reduce my stress and prevent continued work burnout from affecting this recovery.

Will continue another 2 week cycle unless you have objections.

Hope you are doing well.

Got to relax more in my new job role and had some activity in the ear with this last round.

Good drainage throughout both weeks.

The pressure/fullness of the whole ear has reduced to just behind the ear where the mastoid lump is.

Q-tips for lightly drying my ear canal are no longer are making me gag.

The area above the mastoid lump seems to have flattened out. Had several blemishes occur under the hair line above the mastoid before this felt flattened out.

Things feel like they might be moving down towards the mastoid process, but no major change in the remaining lump. I would estimate the lump to be about 2cm across and possibly 1 to 1.5 cm tall at this point (very hard to gauge).

Please advise if you think we're okay to continue.

Dr Basu,

Very well thanks to you. It took me almost a week to get the medicine, so I'm only on day 6 of the first week. (I also bridged the week gap with another 200c dose)

I initially noticed another flattening sensation in the region behind the mastoid towards the back of the head. This now all feels completely normal. I have had a little bit of swelling at the base of the back of the head (top of the neck), but some lymph massages seem to be keeping it controlled and at bay.

The week has continued with an amazing amount of sinus drainage from the ear along with water sensations inside the ear canal. Enough drainage to where I've been coughing most of the week (productive).

My wife and I have both noticed a reduction in the swelling of the knot overall. Not a drastic reduction, but a positive one and thrilled to see it moving.

Hoping week two will bring more positive results and will report when it completes.

Again - I cannot thank you enough for you help!

Greetings Dr Basu,

Second week has continued well. Additional improvements in size of the mastoid resulting in good movement in the main knot. Best way to describe is that the height of it has dropped to about 1cm and the top side seems to be receding faster causing the once dome like shape to now be slightly concaving.

The sinus drainage wasn't as intense as the first week, but still seemed consistent. Perspiring more easily on the left side of the head at times where all the swelling had been. Some nights of head perspiration dampening my pillow.

Your advise would be greatly appreciated in if we're okay to continue.

Greetings and hope you are doing well.

Continued improvement over the last two weeks with some height reduction (about 2 mm reduction). The sides of the knot seems to be receding well into the surrounding bone making it very hard to judge the base of the knot (being absorbed).

The softer tissue region below the ear and mastoid process, which had been a little harder, has softened up substantially. Trying to keep up with ear and lymph drainage massages in this area.

Feeling better overall lately too. Please let me know your thoughts. Thank you.

Continuing well here. No major change in peak height, but some reduction surrounding the knot. Starting to feel the bone getting back to normal in the surrounding area. Sinus drainage continues with varing intensity.

Dr. Basu,

Hope this finds you well.

Week progressed well. Some very limited fullness in the ear (between the ear and the mastoid knot). Sinus drainage was not as prevalent, but still continued with varying intensity (decreased rate and more thick/stringy). I've been waiting for the fullness feeling to increase more, but it seems to be holding where its at for now. Peak height is still right under 1 cm.

Is it possible that we can expect there to be some permanent "stretching" damage to the air cells and the area may never fully return to its original size? I've also wondered if I should pursue getting a CT to see how things are going. Open to all the advise you have on the matter.

Please do the following:
1.CT scan for temporal bone
2.Complete Blood Count.
Apart from this take:
Hepar Sulphur 10M on every 7th day for next month.and feedback thereafter.
