having constant dizziness

Hi,
I am a 37 year old male. I am having constant dizziness throughout the day for last 16 months. It is a forward plus right pull. Dizziness started after a food poisoning in May 2016. After that, in Aug 2016, I got blockage of right ear and tinnitus in right ear. Since then my sleep was very bad. In April 2017, sleep became near normal and tinnitus exists, but is not a major disturbance. Dizziness reduced (never went away), but since last week it has increased though less than what it was even 2 months back. When I walk on a road which has a rightward slope, I can feel some discomfort like being pulled to the right. I have not driven since getting the dizziness and it is almost 18 months that I am not able to drive.
