Sudden extreme fatigue

I am getting sudden bouts or flare ups of extreme fatigue and brain fog.They come out of the blue accompanied by leg pain in knees and calves, low level dull pain. I have had lab tests for endocrine and rheumatoid profiles. All are normal except of ANA test which is 1:160 speckled. They bouts are very debilitating and last from a week two weeks or so. Otherwise , aim in general good health. Sleep does not alleviate fatigue. I am a 42 yr old female. Any expertise on this matter is greatly appreciated. Does anyone have any idea what is causing this?
