I am extreamly deepressed and upset. Please pay attention to help me. please...

I have been encountring semenal discharge (dead sperm found in urine test I took in 2006) that occur during urination mixed within it that is whenever i pee it contain semenal fluid which urine test report as dead sperm. I also encounter dribbling of urine drops that I encounter periodically after I finsh peeing. I am a unmarried male and 24 years old. Graduated and jobless since 2 years. These problems are due to engagement in m*stu*bat*on and p**n addiction that I did since I was 13 and still doing it even I try to resist me. But now I have decided to leave this forever. This made me extreamly weak and now I am unable to do daily routine tasks specifically that invlove physical engagement. I am also weak in general. I feel very discomfortable after I pee and mostly feel inflamation during and after it. Can't do much typing on keyboard typing because arms starts pain. I have also pain in joints and muscle both in (mostly legs) and arms. Pains and weakness get at extreame when go to bed for sleeping which got difficult to . I am in severe trouble. I have been careless about my health and due to fear of side effects of allopathy, homeopathy and herbals that I have heard from people about there problems. But now I am ready to take any medicine so I can get recover quickly. Since previous 4 days I am feeling testical pain specifically in left one.
My weight is 50Kg and height is 5'-5'. I am mentally dipressed from my physical condition at this early age as it looks that I am over 60 as age of 24. My memory is getting weaker with time and got periodic loose motion with pain in stomach when take some kind of mental stress. Due to extreme weakness and shivering of hands I feel guilty and try to to be in isolation and can't maintain my self to sit in public for more than 15 minutes. I use to sit in front of computer the whole day as i am a computer personale. I am upset about my health and future. Please if someone can help to give me sincere medical advice which has no fear of side effects I will be thankful for ever.
