anal fissure

I am a 45 yr old male recently diagnosed w/anal fissure located at 6:00 position w/a skin tag. The fissure is extremely painful(burning & deep throbbing pain) all the time, esp after a BM (once daily upon waking). I do not have any bleeding or discharge. I've had external hemorrhoids for 20 years. I am healthy, very physically active and eat well. I use Arnica several times/year for work and sport related injuries. Arnica has not helped with the fissure. Perhaps because I don't understand dosage rates well. Thank you in advance for your assistance!
