Doctor's help needed - sexual problems

Hello Doctor,
I'm 25 yrs old male with 68 kgs weight and 5'9 inch height.
I have the following problems.
I ejaculate very soon for the past 2 years. within 5-10 secs.
I have sunken eyes with dark circles in it.
I do have night discharge once or twice a week.
I also have urge to urinate very frequently sometimes.
Please Doctor, help me with the treatment.
